Napol Dental Hobart Pty Ltd, trading as Hobart Dental & Implant Centre ("we", "us"), is bound by the Privacy Act 1988 (Cth) and the Australian Privacy Principles. This policy explains how we handle your personal information, including health information.
What we collect
We collect information needed to provide dental care and run our practice, including your name, contact details, date of birth, Medicare, health fund and DVA details, medical and dental history, medications, clinical notes, radiographs and scans, photographs, payment information, and correspondence with you and other health providers.
How we collect it
Usually directly from you — in person, by phone, by email, through our online booking system or website forms. We may also collect information from other health practitioners (for example a referring dentist or radiology provider), your health fund, or a person responsible for you, with your consent or where permitted by law.
How we use it
To assess and treat you; communicate with you about appointments and treatment; process health fund claims and payments; prepare documentation you request (for example for early release of superannuation); communicate with other practitioners involved in your care; meet our legal and professional obligations; and improve our services. With your consent, we may send you practice news; you can opt out at any time.
Disclosure
We may disclose your information to other health practitioners involved in your care, dental laboratories, radiology providers, health funds and HICAPS, finance providers you apply to, our IT and booking-system providers, and government bodies where required or authorised by law. We do not sell your information. Some service providers may store data outside Australia; where this occurs we take reasonable steps to ensure it is protected.
Clinical photographs
We take clinical photographs as part of your records. We only use photographs on our website or social media with your separate written consent, which you may withdraw at any time.
Storage and security
Records are held in secure electronic systems with access controls. We retain health records for the periods required by law and dispose of them securely.
Website
Our website uses cookies and analytics tools (such as Google Analytics and advertising pixels) to understand how the site is used and to measure advertising. These collect technical information such as pages visited and device type. You can control cookies in your browser settings. Please do not include detailed health information in website forms.
Access and correction
You can ask to access or correct the information we hold about you by contacting us. We will respond within a reasonable time. A fee may apply for copying records, which we'll tell you about first.
Complaints
If you have a concern about how we've handled your information, please contact us first. If you're not satisfied with our response, you can contact the Office of the Australian Information Commissioner (oaic.gov.au) or the Tasmanian Health Complaints Commissioner.
